401(k) match by company
Every one of the 57,824 large 401(k) plans on file with the Department of Labor has a page here, 56,422 employers in all. For 219 of those employers the match formula is on file word for word, because they file an SEC Form 11-K and that is the one document that states it. This page lists those 219, with the vesting schedule, eligibility and plan facts from the same filing, every number linked to the sentence it came from.
For every other employer the source is the Form 5500 its plan files with the Department of Labor, which does not carry the formula. Those pages give what the employer paid in per person, what the plan cost per person, where both sit among plans of the same size, and who keeps the records. Browsing by state or by industry reaches every one of them.
That data covers plans with 100 participants or more. A smaller plan files a shorter form that reports far less, so a small employer is missing from the source itself and not waiting to be added.

Who matches the most, and who matches the least?
181 of these 219 filings state the match as a percentage of pay. Across those 181, the maximum employer match runs from 0.25% of pay to 10%. The other 38 state something a percentage cannot carry: a rate set at the company’s discretion each year, terms set by a collective-bargaining agreement, a cap in dollars, no match at all, or a formula the filing never spells out.
